The site, called Healthcare.gov, was launched despite multiple warnings by those overseeing the project that the system just was not ready for "prime time."
And the $400 million sign-up system began to crash almost immediately.
The New York Times traced those flaws to the administration's inability to manage more than 50 contractors who were tasked with cobbling together the system in a short amount of time.
